This plan is for uninsured individuals and may not be used as a supplemental or secondary insurance plan. To be eligible for participation, you cannot be covered by any other dental insurance plan. This is an in-house membership plan and is NOT dental insurance. It is a membership program for prepaid dental services offered by your dentist for your use at Jacksonville Gentle Dentistry. The Plan discounts are off of usual and customary fees and do not cover procedures performed outside of our practice or procedures performed by specialists and can not be combined with insurance or other discounts.
