This is NOT a Dental Insurance Plan. It's an in-house discount plan that uninsured patients of this office can join to save on dental care they receive at this office only. This discount plan can not be combined or used in with insurance. Discounts provided are off usual and costumery fees,(UCR)fees. Visit office for additional information.
